Clinical Liaison  

ALIYA Healthcare is seeking a dedicated Clinical Liaison to serve as a key connection between residents, families, and healthcare providers. This role is responsible for facilitating communication, supporting care coordination, and ensuring residents receive the appropriate services and resources throughout their stay. 

Key Responsibilities  

• Serve as a primary point of contact for residents, families, and healthcare providers 

• Facilitate communication between clinical teams, physicians, and support staff 

• Assist with coordinating resident care plans and services 

• Identify resident needs and connect them with appropriate internal or external resources 

• Support smooth transitions of care and continuity between providers 

• Provide education and guidance to residents and families regarding available services 

• Monitor resident progress and communicate updates to the interdisciplinary care team 

• Ensure all documentation and processes comply with regulatory and organizational standards 

Qualifications  

• Clinical background required) 

• Minimum 2 years of experience in skilled nursing or long-term care required 

• Strong interpersonal and communication skills 

• Ability to manage multiple priorities in a fast-paced healthcare environment 

• Strong organizational and problem-solving abilities
