Description

• Conduct thorough evaluations for patients in inpatient and outpatient settings while crafting personalized treatment strategies
• Implement physical therapy interventions concentrating on mobility, strength enhancement, balance recovery, and overall functional improvement
• Work collaboratively with nurses, doctors, case managers, and fellow rehabilitation professionals to achieve optimal patient outcomes
• Educate patients and their families on therapeutic goals, transitioning plans, and recommended home exercises
• Maintain meticulous documentation of evaluations, treatments, and patient progress in line with regulatory and facility standards
• Engage in collaborative care team meetings and interdisciplinary conferences to ensure a well-coordinated approach to patient care

Education

• Earned a Doctorate in Physical Therapy (DPT) or an equivalent credential from a recognized academic institution

Certifications

• Required state licensure as a Rehabilitation Therapist / Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., geriatric, neurological) are valuable but not mandatory
