Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs

Admissions of patient for services and development of the initial patient/family Plan of Care

Uses health assessment data to determine nursing diagnosis

Coordination of services with the patient, family, and hospice interdisciplinary team members

Valid Registered Nurse (RN) license

RN Diploma, Associates or B.S.
