Remote Case Management Coordinator for Medicaid LTC
VIVA
VIVA USA Inc. is seeking a Case Management Coordinator to coordinate care for Medicaid Long Term Care enrollees. The role involves telephonic and in-person collaboration to plan, implement, and monitor care while improving member health outcomes.
The position requires case management experience and a background in Long Term Care, with strong MS Office skills (Excel, Teams, Outlook). Fully remote with standard M-F hours may be offered.
