What Youll Do

• As a Hospice Admissions RN you play a critical role in guiding patients and families through the start of their hospice journey
• Conduct pre admission evaluations to determine eligibility and appropriateness of care
• Educate patients and families about hospice services with compassion and clarity
• Obtain informed consent and initiate the admission process
• Perform comprehensive nursing assessments and develop individualized care plans
• Deliver interventions that maximize comfort dignity and quality of life

What You Bring

• Graduate of an accredited RN program
• Active Registered Nurse license in the state of practice
• Current CPR certification
• Hospice experience preferred not required
• Strong clinical judgment and ability to work independently
• Compassionate communication skills with patients families and care teams
• Understanding of quality assurance and patient care standards
