Remote Care Coordinator & Health Coach
Cosan Group
Cosán Group is seeking a compassionate Care Coordinator to support a panel of patients from a HIPAA-compliant home office. This 100% remote role focuses on health coaching, outreach, and care coordination with clinical teams to close gaps in care. You will handle 400–450 patient panel, perform daily calls, document interactions in the care coordination platform, and ensure compliance. Must have MA-C or CPhT certification and EMR experience; remote work requires reliable broadband.
