Registered Nurse (RN)-Case Manager

Overview The Health Resource Manager is responsible for providing case management services for SVMC patients. Responsibilities Complete intake interviews to identify a holistic plan of care; facilitate multiple aspects of care coordination with the patient, family/caregiver, providers and other community resources prior to discharge Develop a patient transition plan, provide patient/caregiver education, and proactively advocate for patient needs and goals across the interdisciplinary team to ensure an approach sensitive to each patient and family’s needs Assist providers in determining the patient’s level of care in accordance with InterQual criteria Actively seek collaboration with providers to improve overall quality and efficiency of care Qualifications Graduation from an accredited school of nursing 2 years of recent clinical experience required A Vermont RN Nursing License is required by the date of employment Current BLS Certificate Prior experience in utilization review, discharge planning and/or case management is preferred but not required Details Area of Interest: Nursing Pay Range: $36.00-$46.00 Work Status: 8:00-4:30p Employment Type: Full Time Job ID: 6296 Benefits Dartmouth Health offers a total compensation package that includes a comprehensive selection of benefits.
