Home Health Registered Nurse

About the Role As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care to patients in their homes, helping them regain independence. Responsibilities Provide skilled nursing care, including medication administration, wound care, and patient assessments. Develop and implement individualized care plans in collaboration with physicians. Monitor patients' conditions and report changes. Educate patients and families on disease management, medication, and treatment options. Maintain accurate records of patient care and coordinate with other healthcare professionals. Report patient care and condition progress to the physician and Clinical Manager. Oversee Home Health Aide work and ensure documentation is completed properly. Required Qualifications Diploma, Associate or Bachelor’s Degree in Nursing. Experience in medical-surgical, ICU, ER, or acute settings. Current and unrestricted Registered Nurse licensure. Current CPR certification. Experience collaborating with a multidisciplinary team. Valid driver’s license, auto insurance and reliable transportation. Preferred Qualifications One year nursing experience. Home Health experience. Compensation $49.00–$69.00 per visit/unit. $77,200–$106,200 per year base pay (full time). Additional Requirements TB screening is required for this patient‑facing role, and you must maintain a valid state driver’s license and vehicle insurance per Humana’s safety program.
