UnitedHealth Group's UnitedHealthcare division seeks a Maternal Registered Nurse Case Manager in Washington state. The role coordinates care for pregnant or recently pregnant Medicaid members, performing home and social assessments and educating members and families. You will work with internal care teams, providers, and community resources to develop person-centered care plans, manage chronic conditions, and promote health and safety.
