Additionally: ensures compliance with Federal, State, and local laws and regulations. May also advise physicians on medication utilization. Responsibilities Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status Advise patient care team on patient's medication therapy and document such interventions. Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency. Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records. Qualifications Education: Pharm. D. or BS in Pharmacy Experience: Preferred: Hospital experience License: Current registered pharmacist license in the State of Florida Certifications: Required: BLS within 30 days of hire (ACLS and/or PALS if code attendance required) Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc. Organization Description St. Marys Medical Center (SMMC), part of the Palm Beach Health Network, is a 420-bed acute care hospital in West Palm Beach. The hospital has been serving Palm Beach County and the Treasure Coast for more than 80 years.SMMC is a Level 1 Trauma Center, a state-designated Pediatric Trauma Referral Center, certified Comprehensive Stoke Center, and has a level III NICU.
