Primary Care Nurse Practitioner
Location: Indiana
Start: ASAP
End: December 31, 2026
Schedule: Monday through Thursday clinic schedule. Typical hours 7:00 AM to 5:00 PM with a one hour lunch. No nights. No weekends.
Openings: 1
Position Overview
- Employer sponsored primary care and wellness clinic
- Scope includes primary care, acute care, and occupational health or work comp
- Adult focused, with occasional patients under age eighteen
- Proactive care, chronic disease management, early intervention, and patient education
- Work autonomously in accordance with state practice requirements
Required Qualifications
- Active, unrestricted Indiana FNP license
- National certification as a Family Nurse Practitioner
- DEA registration
- Experience treating patients age two and older
- Strong HIPAA and confidentiality standards
Credentialing and Onboarding Items
- Ability to pass on site credentialing
