Position Summary Provide case‑management services and support to individuals requiring assistance with activities of daily living (e.g., bathing, dressing, toileting, eating, mobility). Core responsibilities include establishing positive relationships, exploring options, developing and monitoring person‑centered plans, meeting state‑mandated requirements, and ensuring provider compliance with standards. Essential Duties Advocate for clients, provide education and resources, and promote self‑determination Collect eligibility information and documentation, work with clients and representatives, maintain updated plans Develop and write person‑centered plans, conduct joint preparation sessions, and continually update tools Research, refer, and coordinate services to meet plan outcomes; assist with transitions to community from institutional settings Monitor plans, conduct visits, track progress, complete required reports, and re‑apply for programs as necessary Adhere to SCI policies: meet performance benchmarks, maintain HIPAA records, use tracking tools Required Competencies Problem solving, decision making, job knowledge, achievement, initiative, trustworthiness, respect, communication, relationship building Service orientation, teamwork, dependability, flexibility, detail orientation, professionalism, planning, stress tolerance, positive work attitude Minimum Qualifications Bachelor’s or Master’s degree in human services field (Human Services Administration, Psychology, Sociology, Social Work, Nursing, Counseling) Experience Experience with people with disabilities (preferred) Experience with person‑centered planning (preferred) Physical Demands Standard office environment; must remain stationary >95% of time, use computer, keyboard, mouse, and mobile device.
