RN Case Manager (Home Health) $37.44-$40.13/hour | Part-Time Baywood Home Care | Fairmont, MN Baywood Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care, support patients and families, and help individuals maintain their independence in the comfort of their own homes.
