Legacy Community Health is seeking a compassionate RN – High Risk OB Care Manager to join our maternal health team. This role provides high‑risk perinatal care coordination, care transitions, and clinical triage for pregnant and postpartum patients with elevated medical or social risk. You will collaborate with physicians, Care Management staff, and Perinatal Navigators to improve outcomes and promote culturally responsive, trauma‑informed care. #J-18808-Ljbffr Legacy Community Health
