Occupational Therapist (OT)

We're seeking a skilled and compassionate Occupational Therapist (OT) to provide high-quality care to patients in home care and hospice settings. This is an opportunity to make a meaningful impact by helping patients improve their independence, comfort, and quality of life.

Responsibilities

• Conduct comprehensive evaluations of patients' functional status and home environment.
• Identify equipment and adaptive needs.
• Develop and communicate personalized treatment plans in coordination with physicians and care teams.
• Teach and retrain patients in daily living activities, adaptive equipment, and prosthetics/orthotics.
• Provide specialized care to hospice and home care patients.
• Maintain accurate clinical documentation and report changes in patient condition.
• Communicate with care team members and supervise therapy assistants as needed.
• Educate patients, families, and caregivers on treatment plans and therapeutic techniques.
• Participate in team meetings and assist with discharge planning.

Requirements

• Degree from an accredited occupational therapy program.
• Registration with the National Board for Certification in Occupational Therapy (NBCOT).
• At least 2 years of clinical experience as an Occupational Therapist.
• Hospice and home care experience preferred.
• Strong verbal, written, and organizational skills.
• Current CPR certification.
• Valid driver's license and reliable, insured vehicle.
• Current Occupational Therapy License.
