>REF:#2a483769fa77b687 Job Summary: The Physical Therapist (PT) provides intermittent physical therapy to patients in various settings, following physician orders and care plans under the supervision of the Regional Director of Therapy, Outpatient. Responsibilities: Assess patient functional abilities and develop treatment plans. Document evaluations and treatment plans for physician approval. Coordinate with the interdisciplinary team and supervise Physical Therapy Assistants. Instruct patients and caregivers on therapy practices. Communicate patient progress and changes to the healthcare team. Requirements: Bachelor’s degree in Physical Therapy, valid state license, and preferred two years of experience. Work Environment: Indoors in various facilities and patient homes, with travel required. #J-18808-Ljbffr AW Health Care
