**Required Experience and Qualifications:** - 1 year of previous experience - At least 1 completed travel contract - Experience with Siemens Edge equipment - Epic experience - Iowa Permit to Practice - State license - Not an employee of UnityPoint in the last 12 months **Required Certifications and Licensure:** - RT(R)(CT) **Required Submissions and Documentation:** - Driver’s License - Work history/resume - Skills checklist (completed within 1 year of submission date) - License verification - Copy of required certifications - References - 1 supervisory reference required at time of submission **Taxable Address Requirement:** - Must have a taxable address outside of 50 miles from the facility.
