Duties Review and verify client documentation to establish eligibility for Medicaid and other health insurance programs Conduct thorough assessments of applicants’ medical and social histories, including hospital experience and behavioral health status Collaborate with case management teams, social workers, and healthcare providers to facilitate discharge planning and coordinate services Utilize electronic health records (EHR) systems for accurate data entry, documentation, and tracking of client information Maintain strict adherence to HIPAA regulations by safeguarding patient confidentiality during all interactions Communicate effectively with clients from diverse backgrounds, including bilingual populations, ensuring clear understanding of eligibility criteria and next steps Qualifications Proven experience working with individuals in healthcare settings, preferably with hospital or acute care experience Familiarity with electronic health records (EHR) or electronic medical records (EMR) management systems Excellent customer service skills combined with compassionate client communication abilities Ability to perform detailed data entry accurately while managing multiple priorities efficiently Knowledge of HIPAA compliance standards and maintaining patient confidentiality at all times Join us in delivering compassionate healthcare support by ensuring that every individual receives the benefits they need.
