This opportunity is primarily based at a 254-bed hospital in Elkhart, Indiana, with potential crossover at a larger regional referral center in nearby South Bend. Lead goals-of-care and advance-care-planning discussions with patients and families. Manage complex symptoms including pain, nausea, fatigue, dyspnea, appetite changes, constipation, and sleep disturbances. Assist patients and families in understanding diagnoses, prognosis, and available treatment options. Support complex medical decision-making while ensuring treatment plans remain aligned with patient goals and values. Collaborate with treating physicians while patients continue disease-directed or curative therapies. Provide emotional, social, and spiritual support in collaboration with the interdisciplinary team. Help facilitate communication between patients, families, specialists, and hospital care teams.
