  • Conduct comprehensive assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors.

  • Provide counseling and emotional support, helping seniors and their families navigate life transitions such as relocation to assisted living or coping with health changes.

  • Work collaboratively with an interdisciplinary team including nurses, caregivers, and healthcare providers to enhance patient care and coordinate services effectively.

  • Perform regular visits to monitor clients’ well-being and living conditions, ensuring safety and addressing any immediate concerns.

  • Facilitate communication and conflict resolution among family members involved in the senior client’s care process.

  • Educate clients and their families about community resources, support groups, and programs that promote social engagement and enhance quality of life.

  • Maintain accurate documentation related to client assessments, care plans, progress notes, and any significant interactions in compliance with regulatory standards.

Patient Population / Client Focus\n

The primary focus of this role is the senior population within the Hialeah community. You will support elderly clients facing various challenges, including mental health issues, social isolation, and difficulty accessing essential health services. You will also provide comfort and guidance to families navigating the complexities of caring for aging loved ones.

Qualifications\n

  • A Master’s degree in Social Work from an accredited institution.

  • Current and valid state licensure as a Licensed Master Social Worker (LMSW).

  • A minimum of two years of experience in social work, preferably in a community health or geriatric setting.

This position offers a competitive salary ranging from $60,000 to $100,000 annually, commensurate with experience and qualifications.
