Care Management Social Worker - PRN (Adult)

Position Title: Care Management Social Worker - PRN (Adult) Department: OUMC Care Management Job Description: The Care Management Social Worker is responsible for managing care for high-risk members with chronic behavioral and health conditions, collaborating with members, caregivers, physicians, and the healthcare team to ensure timely access to necessary care, continuity across all settings, shared decision-making, and connections to supportive services and community resources. Under a Social Worker II, they offer essential support and guidance to patients and families as they navigate wellness within the OU Health. The Social Worker II works closely with the interdisciplinary care team to coordinate patient care across the continuum, ensuring access to adequate resources and services.

PRN POSITIONS REQUIRE A MINIMUM OF 24HRS A MONTH

Essential Responsibilities Responsibilities listed in this section are core to the position. Assess assigned patients’ unique conditions by combining data from medical diagnoses, chart reviews, personal interviews, and care teams. Provide social services to patients and their families for an assigned medical service. Provide or supervise plan of care interventions to include resource and referral help, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling as appropriate, partnering with the healthcare team and involving the patient and family in the development and implementation of plans. Provide patients and family education regarding diseases, treatments, social or financial situations, emotional stress and/or other problems. Monitor care to evaluate progress towards goals and institute appropriate interventions as needed. Conduct high risk screening and provide crisis counseling, education and resources to patient and families regarding abuse or neglect, physical abuse, etc. and document these sessions. Act as a liaison between patients/families and the clinical staff and other agencies. Advocate for patient concerns and elevate ethical dilemmas through the appropriate channels. Complete applicable reports based on mandated reporting requirements. Precept newly hired staff members, providing field instructor services to schools of social work to train bachelor and master’s program interns to the hospital or health care setting. Drive performance improvement initiatives to include data collection and research, development, and participation in continuing education programs. Maintain an open, approachable manner treating others fairly and respectfully, preserving self-confidence and dignity, and showing regard for their opinions. Lead Care Management team meetings and interdisciplinary rounds. Assist with projects as assigned. Complete Leadership academy as assigned. Commit to continuing education and professional development. General Responsibilities Perform other duties as assigned. Minimum Qualifications Education Requirements: Master of Social Work from an accredited institution required. Experience Requirements: At least 3 years of social work experience. License/Certification/Registration Requirements: Current Licensed Master Social Worker (LMSW) or under supervision for Licensed Clinical Social Work (LCSW) or LCSW from the Oklahoma State Board of Licensed Social Workers. Current Basic Life Support certification from the American Heart Association required. Knowledge/Skills/Abilities Required: Knowledge of the principles and practices of casework. Knowledge of community resources. Analytical skills sufficiently strong to interpret situations and determine the appropriate course of action. Ability to explain information in a way that patients, families, and staff can understand. Good interpersonal skills. Strong knowledge of healthcare regulations, including CMS guidelines. Serve as liaison between patients, families, and healthcare providers. Strong communication, interpersonal, and leadership skills. Detail-oriented with excellent organizational skills. Commitment to foster a culture of continuous learning, quality improvement, and patient-centered care. Strong assessment, critical thinking, and problem-solving skills. Ability to assess, diagnose and treat mental health conditions through providing counseling services. Basic leadership skills and knowledge. Strong ethical standards and professional integrity; knowledge of Code of Ethics. Ability to handle emotionally charged situations with compassion and professionalism.
