RN/LPN active license

Must have valid CPR BLS card.

Must have a copy of a neg TB skin test results, chest Xray or TBQ gold Quantiferon on blood test results.

Must have Level 2 background clearance.

Must be able to provide proof of car insurance.

Must speak English clearly.

Must have good driving record.

Must have reliable transportation.
