Licensed Practical Nurse

REM Community Services, part of the Sevita family, provides community‑based services for individuals with intellectual and developmental disabilities. This full‑time Licensed Practical Nurse position offers a pay rate of $20.40 per hour, a welcome bonus of $3,000, and opportunities to support and train individuals with brain injury, intellectual and developmental disability, substance use, and behavioral challenges. Responsibilities Monitor health conditions of individuals served and ensure quality services are provided. Administer medications, provide documentation, and oversee staff who administer medications. Provide prescriptive medical treatment as ordered by physicians and deliver personal care services. Conduct assessments and implement health‑care services to be included in each Individual Service Plan, responding to changes in condition. Provide training to staff on universal precautions, infection control, and medication administration. Serve as liaison and advocate with other medical and professional service providers. Provide training for individuals served on topics such as self‑medication, prescribed treatments, sex education/STD prevention, and smoking cessation. Qualifications Associate’s degree in Nursing or related field, or graduate of an accredited LPN program. One year of experience working with the population served in a nursing capacity. Current state LPN or LVN license. Current driver’s license, car registration, and auto insurance. Additional certifications required by state licensing regulations, such as CPR/FA, HIV/AIDS certification, BBP, and ACT training.
