RN Case Manager: Discharge & Care Coordination

The RN Case Manager role at Cynet Health is a 13-week contract based in Illinois. You will oversee discharge planning and care coordination for an assigned caseload of 13-25 patients in an acute hospital setting. Strong clinical judgment and collaboration with a multidisciplinary team are essential.
