About the Role: We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team! As a Licensed Practical Nurse, you will play a vital role in providing exceptional, person-centered care to our residents and patients. Responsibilities: Provide direct patient care to residents and patients, assisting with activities of daily living under the direction of a professional nurse. Assist in the implementation of individualized treatment plans for each assigned resident. Supervise CNAs and participate in their evaluation. Oversee Falls Program and Pressure Sore Program. Provide nursing services and conduct thorough evaluations of each resident’s medical status upon admission and throughout their course of treatment. Requirements: A current, unencumbered, active state license to practice as an LPN. Experience in Skilled Nursing/Rehabilitation facilities preferred.
