If so, apply today!As an **Clinical Team Manager**, you’ll contribute to our success in the following ways:* Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are consistently met.* Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.* Ensure timely start of care and ongoing compliance with applicable regulatory requirements.* Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.* Conduct regular OASIS reviews to support accurate documentation, compliance, and quality patient care.* Provide guidance and coaching to direct care team members and assist with interviewing and hiring clinical staff.* Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.**What is Required?*** Associate degree in a Nursing related field required* 2 years home care experience as a registered nurse in a Home Health or Hospice environment* 1 year of supervisory experience preferred* Current, unrestricted RN license valid for the state of work* Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment* Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.**What's in it for you?*** Comprehensive insurance plans for medical, dental, and vision benefits including pet insurance* 401(K) with employer match* Tuition reimbursement for full-time staff and continuing education opportunities for all employees at no cost* Paid time off, paid holidays, family, and pet bereavement* Fleet vehicle program (restrictions apply) or mileage reimbursement.\\* Benefit eligibility can vary by position and shift status. ***Equal Employment Opportunity**: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
