About the Role:

We're seeking a dedicated and skilled LPN to join our team of healthcare professionals at Elizabeth Park Rehabilitation and Nursing in Elizabeth, New Jersey. As an LPN, you'll work under the supervision of a licensed physician or RN to provide direct patient care, administer medications, and perform various medical procedures. Your expertise will ensure that our residents receive the highest quality care, tailored to their unique needs and preferences.

Responsibilities:

• Provide direct patient care, including vital sign monitoring, wound care, and medication administration

• Assist patients with activities of daily living (ADLs) such as bathing, dressing, and grooming

• Perform various medical procedures, including injections, venipuncture, and phlebotomy

• Maintain accurate and detailed records of patient care and treatment

• Collaborate with healthcare team members to develop and implement individualized care plans

• Participate in quality improvement initiatives to optimize patient outcomes and facility performance
