- Conduct patient assessments to identify nutritional needs and develop personalized meal plans
- Collaborate with healthcare team members to ensure effective communication and coordination of care
- Provide nutritional counseling and education to patients and their families
- Monitor patient progress and adjust nutrition plans as needed
- Develop and implement nutrition programs to promote patient well-being and quality of life
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Maintain accurate and timely records of patient care and nutrition plans
- Bachelor's degree in Nutrition or Dietetics
- Registered Dietitian (RD) credential
- Experience in acute care, rehabilitation, or long-term care settings (2 years of experience minimum)
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
