LTC/SNF LPN

Position Overview

The Long-Term Care (LTC) / Skilled Nursing Facility (SNF) Licensed Practical Nurse (LPN) plays a critical role in providing comprehensive care to residents in a healthcare setting. This position requires a commitment to delivering high-quality care to elderly patients, including those with complex medical needs and behavioral health challenges.

Job Details

• Shift Length: 12 hours
• Guaranteed Hours: 36 hours per week
• Years of Experience Required: 1 year
• Weekend Requirement: Every other weekend
• Holiday Requirement: Must work holidays that fall within scheduled block

Responsibilities

As an LPN in this setting, your responsibilities will include:

• Administering medications and monitoring patient responses.
• Performing routine assessments and documenting observations in the electronic medical record (EMR).
• Collaborating with nursing staff and other healthcare professionals to deliver coordinated care.
• Participating in the development and implementation of patient care plans.
• Assisting with activities of daily living to ensure the comfort and well-being of residents.
• Providing support to patients and their families regarding care-related issues.

Qualifications

• Current Licensed Practical Nurse (LPN) licensure.
• BLS Certification is required.
• Prior experience in a long-term care or skilled nursing facility is preferred.

Work Environment

This role involves working with an elderly population that includes residents with dementia and other behavioral health conditions. The facility is designed to support both physical and mental health needs, creating a therapeutic environment for all residents. The LPN will work closely with a medication aide and nursing staff to ensure a high standard of care is maintained.

Orientation and Training

New hires will participate in a comprehensive orientation program that includes training, facility tours, competencies, and opportunities to shadow experienced staff during their initial shifts.
