Licensed Practical Nurse (LPN) - Skilled Nursing Facility

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in a Skilled Nursing Facility (SNF) setting. This role is primarily focused on providing high-quality patient care for both long-term residents and those recovering to regain their strength.

Responsibilities

• Administer medications and assist with wound care.
• Provide foley/catheter care, PEG tube feedings, ostomy care, and tracheostomy care.
• Support residents in daily living activities and maintain a safe, compassionate environment.
• Collaborate with RNs regarding patient care, including IVs, PICC lines, and mediports.
• Document patient progress and care provided using the electronic medical record system.

Schedule

This is a 12-hour day shift position, with shifts typically from 6:45 AM to 7:15 PM. Weekend work is required every other week, with a guaranteed minimum of 48 hours per week.

Required Experience / Certifications / Licensure

• Current and valid LPN license.
• CPR certification required prior to the first day of work.
• Experience in skilled nursing and long-term care settings preferred.
