Job Summary:
Pharmacist Inpatient (PHARM IP) in a hospital setting. This role involves overseeing pharmaceutical services, ensuring medication safety, preparing and dispensing medications, and participating in clinical and operational activities within the inpatient department. The position emphasizes patient safety, medication management, and collaboration with healthcare professionals to provide high-quality care.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Successful completion of all educational requirements mandated for state licensure, current State licensure, Basic Life Support (BLS) certification if required by State, EPIC experience, and a minimum of two years of recent hospital experience.
• Preferred Qualifications: Experience with medication reconciliation for discharge, strong communication skills for patient and family education, and clinical knowledge in antimicrobial stewardship, pharmacokinetic dosing management, and anticoagulation dosing monitoring.
• Additional Information: Maintains adequate drug supply at the pharmacy, follows all federal and state pharmacy laws, manages inventory, prepares sterile and non-sterile medications, documents all transactions, inspects pharmacy areas regularly, manages controlled substances, and participates in quality assurance programs. Develops educational programs for staff and patients, supports medication safety initiatives, and promotes continuous quality improvement within the pharmacy operations.
