Registered Nurse-Wound Clinic Full Time Days

$34.5 - $55.2 per hour

The salary range for this position is $34.50 - $55.20 per hour. Placement within the salary range is dependent on experience, internal equity, and collective bargaining agreements for union positions. A registered professional nurse assumes accountability and responsibility for the individualized, age‑specific care of a patient along the continuum from admission to discharge. The nurse gives direct care to assigned patients and assists with other patient care when necessary. The nurse’s decisions and actions on behalf of the patient/family are determined in an ethical manner. The nurse collects and analyzes assessment data to determine nursing diagnoses, collaborates with the patient/family and other health‑care team members, develops a plan of care to attain realistic outcomes, provides education founded on evidence‑based practice, and systematically evaluates the quality and effectiveness of nursing practice. Responsibilities Assess and record patient status and identify patient care needs upon admission or point of care entry. Organize assessment health data so that it is accurate, complete and accessible. Identify problems that need immediate attention. Validate diagnoses with patient, family and other health‑care team members. Document diagnoses in a manner that facilitates determination of expected outcomes and plan of care. Initiate a problem list at the time of admission and update as needed; incorporate principles of age‑specific care into the patient’s plan of care. Document that the patient’s individualized plan of care has been discussed with and agreed upon by the patient or family. Begin discharge planning at time of admission; consult with patient, family and health‑care team members regarding discharge plan. Identify patient teaching needs on admission and throughout hospital stay. Arrange conferences with patient and/or family and other members of the health team when indicated to plan care. Prioritize and organize nursing interventions that are evidence‑based and reflect established standards of care. Provide individualized, mutually agreeable patient and family education founded on evidence‑based practice and research guidelines. Document all assessments, interventions and patient responses. Consistently review the patient’s health data to keep the plan of care updated. Ensure that revisions to the plan of care are systematic and ongoing and are driven by measurable realistic goals. Document the patient’s progression toward identified outcomes. Identify unmet outcomes and provide direction for continuity of care. Reassess nursing actions for effectiveness in achieving desired outcomes. Communicate with patient, family and health‑care team members regarding patient care and nursing’s role in the provision of care. Collaborate with patient, family and health‑care team members regarding the patient’s progress and plan of care. Present relevant patient information at multidisciplinary meetings and conferences for feedback from peers. Make appropriate referrals as needed for continuity of care. Required Qualifications Bachelor of Science in Nursing (BSN). Current State of Illinois Registered Professional Nurse license. American Heart Association Basic Life Support (BLS). Strong interpersonal, collaboration and analytic skills. Associate Degree Nursing (ADN) acceptable with commitment to obtain a BSN from an accredited program within three years of first day of employment. Additional requirements for the Emergency Room: ACLS, PALS/ENPC and TNCC within one year of hire. Additional requirements for the Pediatric Emergency Room: Pediatric Advanced Life Support and TNCC within one year of hire. Additional requirements for Non‑Invasive Cardiology: ACLS (preferred). Preferred: Trauma Nurse Specialist (TNS). Preferred: Emergency Communications Registered Nurse. Preferred: Certified Pediatric Nurse. Preferred: Certified Emergency Nurse. Preferred Qualifications Experience in a specific clinical area.
