Schedule: Monday Friday, 8:00 AM 5:00 PM
Patient Volume: patients per day
Practice Model: Outpatient only, value-based care, no hospital responsibilities

Ideal Candidate

• Board Certified/Board Eligible in Family Medicine or Internal Medicine
• Experience caring for Medicare Advantage patients
• Comfortable managing complex chronic conditions such as diabetes, CHF, COPD, obesity, and cardiovascular disease
• Familiarity with ICD-10 coding and STAR quality measures
• Experience with eClinicalWorks EMR is a plus
• Adept at working Independently
• Must be Bilingual English/Spanish
