Licensed Practical Nurse (LPN)

Facility Location Care Centers of Nassau 95146 Hendricks Rd Fernandina Beach, FL 32034 Position Summary The Licensed Practical Nurse is responsible for providing and coordinating comprehensive nursing care to individuals residing at Care Centers of Nassau. This role ensures the delivery of high-quality medical care in compliance with physician orders, state regulations, and facility policies while promoting the health, safety, and well‑being of individuals served. Essential Duties and Responsibilities Provide direct nursing care including assessments, treatments, and medication administration in accordance with physician orders and care plans. Monitor and document residents’ health status, vital signs, and changes in condition. Administer medications and treatments accurately and in compliance with all regulations and facility policies. Coordinate with physicians, interdisciplinary teams, and healthcare providers regarding resident care and treatment plans. Develop, implement, and update nursing care plans based on resident needs. Respond to medical emergencies and provide appropriate interventions as needed. Ensure proper documentation in medical records in a timely and accurate manner. Supervise and provide guidance to direct support professionals (DSPs) and other support staff as needed. Ensure compliance with infection control, OSHA, HIPAA, and all applicable federal and state regulations. Monitor medication storage, administration records, and controlled substances in accordance with policy. Participate in care plan meetings, staff meetings, and interdisciplinary team discussions. Educate staff and residents as appropriate regarding health conditions and care needs. Maintain professionalism, accountability, and ethical nursing practice at all times. Attend required trainings, in‑services, and continuing education programs. Perform other duties as assigned. Qualifications Required Qualifications Current and valid Registered Nurse (RN) license in the state of Florida. Graduate of an accredited nursing program (Associate or Bachelor’s degree in Nursing). Basic Life Support (BLS) certification required. Knowledge of nursing practices, medical terminology, and patient care standards. Ability to document clearly and communicate effectively with staff, residents, and healthcare providers. Preferred Qualifications Experience in medication administration and care coordination in a clinical setting preferred. Physical Requirements & Working Conditions Frequent standing, walking, bending, lifting, and moving throughout the shift. Ability to lift up to 30 pounds. Exposure to communicable diseases, bodily fluids, and medical environments. Ability to respond quickly to emergencies and high‑stress situations. Must maintain professionalism and appropriate clinical appearance at all times. May be required to work weekends, holidays, and varied shifts.
