You'll evaluate patients, develop personalized treatment plans, and help individuals regain independence with daily activities while collaborating with physicians, nurses, and the interdisciplinary care team.ResponsibilitiesPerform comprehensive occupational therapy evaluationsDevelop and implement individualized treatment plansHelp patients improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs)Evaluate upper extremity strength, coordination, cognition, perception, and functional mobilityRecommend adaptive equipment, assistive devices, and home modifications to improve safety and independenceEducate patients and caregivers on therapeutic techniques, fall prevention, energy conservation, and home safetyCollaborate with physicians, nurses, physical therapists, speech-language pathologists, and other members of the care teamComplete timely and accurate documentation using KanTime EMRParticipate in interdisciplinary case conferences and care planningQualificationsBachelor's, Master's, or Doctorate in Occupational Therapy from an accredited programCurrent Occupational Therapist license in New HampshireStrong communication and interpersonal skillsExcellent organizational and time management abilitiesComfortable using electronic medical records (EMR)Home health experience is preferred but not requiredWe welcome Occupational Therapists from acute care, inpatient rehabilitation, outpatient therapy, skilled nursing, long-term care, or other clinical settings who are interested in transitioning into home healthBenefitsCompetitive hourly compensationMedical, dental, and vision insurance401(k) with a 4% company matchMileage reimbursement20 days of Paid Time Off (PTO)5 paid sick days8 paid holidays annually (6 fixed holidays + 2 floating holidays)Company-issued iPad and equipmentOngoing mentorship and professional developmentSupportive, clinician-led culture focused on work-life balanceApply TodayIf you're passionate about helping patients regain independence and improve their quality of life while enjoying the flexibility and autonomy of home health, we'd love to hear from you.
