Shift Detail: 8am-4:30pm, Mon -Friday Seymour and Oxford area Cover 1 weekend per month, 1 major and 2 minor holidays per year

Job Description

• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
• Utilize state-of-the-art technology to document skilled services provided.

Qualifications

• Licensed Practical Nurse with Valid CT LPN license
• One year of LPN experience; Homecare experience preferred.
• Valid driver's license and reliable transportation required.
• Computer proficiency required.
