Nurse Practitioner

Job Overview Location: Lakeland, Florida. Reports to an associate medical director. We are looking for a nurse practitioner (NP) or physician assistant (PA) to provide care to seniors as part of a multidisciplinary team. Role & Responsibilities Provide comprehensive care to seniors following clinical standards. Collaborate daily with your care team to ensure coordinated, high-quality care. Manage referrals, hospital/SNF coordination, DME, and home health services. Participate in daily huddles. Document care with support from quality-based coders. Participate in on-call rotation and support growth initiatives at your center. Ensure compliance with all licensing and accreditation requirements. Required Qualifications Master's Degree as a nurse practitioner or completion of a physician assistant program with board certification. Active, unrestricted NP or PA license in the state of practice. Two years of experience in value-based care, geriatric or primary care, managing adults aged 65+ with chronic conditions. Computer proficiency (EMR, email, etc.). Role is patient-facing and part of a tuberculosis screening program; TB screening required if selected. Preferred Qualifications Experience in Medicare Advantage settings. Active DEA license and Medicare/Medicaid provider numbers or ability to obtain them. Familiarity with HEDIS quality indicators and Medicare guidelines. Additional Information Scheduled Weekly Hours: 40 hours per week. Pay Range: $123,800 - $170,300 per year.
