Setting: Skilled Nursing Facility
Schedule: 15 hours/week
In-House Physical Therapist Woodway Rehab & Healthcare in Waco, TX.
Position Overview
As a Physical Therapist , you will evaluate residents, develop personalized treatment plans, and provide skilled therapy services aimed at improving mobility, strength, balance, and functional independence.
Key Responsibilities
- Evaluate residents’ physical conditions and functional limitations
- Develop and implement individualized treatment plans using evidence-based practices
- Provide therapeutic interventions to restore mobility and reduce pain
- Monitor resident progress and adjust treatment plans as needed
- Educate residents, families, and caregivers on exercises and mobility strategies
- Complete accurate and timely documentation per facility and regulatory standards
- Collaborate with the interdisciplinary team to ensure optimal resident outcomes
- Maintain compliance with all safety and regulatory requirements
Qualifications
- Active Texas Physical Therapist (PT) license
- In good standing with all licensing boards and regulatory agencies
- SNF or long-term care experience preferred
- Familiarity with Net Health documentation system a plus
- Strong communication and clinical skills
