CommonSpirit may ultimately pay more or less than the posted range as permitted by law.As our Physical Therapist, you will help Hospice/Home Healthcare patients perform all phases of physical therapy evaluation, treatment planning, treatment, and documentation so they can achieve their rehabilitative goals and improve their quality of life in a home setting.Every day you will evaluate patients, participate in treatment program planning, and administer physical therapy modalities, procedures, and tests.To be successful in this role, you must have at least two years of professional experience in rehabilitation, excel at documenting patient progress and communicating with the medical team, and effectively supervise Certified Home Health Aides.Perform patient initial and re-evaluations, establishing and implementing effective treatment plans.Administer treatment plans using accepted therapy procedures, overseeing Certified Home Health Aides as therapist extenders.Teach patients and/or caregivers functional activities and treatment techniques of physical therapy as appropriate to meet their needs.Document daily on appropriate forms using clear and concise data, recording treatments accurately and communicating patient status/progress in Patient Care Conference and DIC summaries, submitting documentation at least weekly.Communicate regularly with other members of the medical team regarding goals, progress, and rehabilitative potential, working as a collaborative treatment team member.Maintain the standards of conduct as promulgated by the American Physical Therapy Association and adhere to the professional code of ethics in the performance of duties.Required:Two (2) Years Of Professional Experience In RehabilitationActive Physical Therapist (PT) license in the State of CaliforniaBasic Life Life Support – Cardiopulmonary Resuscitation (BLS-CPR)Driver's License: CaliforniaAuto InsuranceMarian Regional Medical Center, a 191-bed facility located in Santa Maria, California, is recognized as one of the Top 250 Hospitals in the Nation by Healthgrades and was awarded Best Maternity Care by Newsweek.
