Physical Therapist

A long-term care facility in Middlesex isseekinga dedicated and flexible Physical Therapist (PT )to join our team on a per diem basis, with the opportunity to make a meaningful impact on residents’ mobility, independence, and quality of life.

Responsibilities:

• Evaluate residents and develop individualized treatment plans
• Provide therapy to improve mobility, strength, and functional abilities
• Collaborate with interdisciplinary team members to supportoptimaloutcomes
• Document patient progressin accordance withfacility standards
• Educate residents and families on treatment plans and exercises
• Ensure compliance with safety and regulatory requirements

Qualifications:

• Active Physical Therapist license in the State of New Jersey
• Prior long-term care experiencerequired
• Strong clinical, communication, and documentation skills

• Flexible availability, including weekdays and weekends
