ICU EXPERIENCE REQUIRED…

1.5+ years experience as a transplant, renal or medical social worker required. Preferred experience at a teaching hospital. In Patient and Out Patient. Provides direct clinical social work services to patients and their families in an interdisciplinary team approach. Business casual. Epic Required. Responsibilities include but are not limited to: Completing initial psycho-social assessments on assigned patients, providing psychotherapeutic counseling with individuals, families, or groups as needed, giving clinical consultations as needed, providing patient‑centered advocacy, collaborating with interdisciplinary teams, managing complex discharge plans, building and maintaining relationships with internal and external resources, making referrals to community agencies and entitlement programs, and continuing ongoing education. Ensuring continuity of care for patients with complex post‑acute needs and maximizing the patients’ and families’ abilities to meet continuing needs within their social and financial framework. Understands all stages of growth and development to assess each patient’s age‑specific needs to provide individual treatment and care. Is responsible for reporting identified safety issues such as hazardous environments (e.g. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors. #J-18808-Ljbffr MED-STAT HEALTH CARE INC
