The Licensed Practical Nurse provides direct patient care under the supervision of a Registered Nurse in a long-term care setting. The role supports the patient care plan, administers medications and treatments, and collaborates with the care team to maintain high standards of quality and safety. This assignment is managed through Healthforce Healthcare LLC.

Responsibilities
- Provide direct nursing care under the supervision of a Registered Nurse, including assistance with personal hygiene and participation in the patient care plan
- Administer medications and treatments, including respiratory therapy modalities, and document administration
- Apply infection control policies to protect patients and staff
- Monitor patients, report changes in condition, and take appropriate action in urgent situations
- Offer patient instruction during hospitalization and at discharge
- Support orientation and ongoing learning for new staff and students

Requirements
- Active Licensed Practical Nurse license (current)
- CPR and ACLS certifications required
- Education: Graduate of an approved LPN program with current licensure
- Visual, auditory and manual dexterity abilities to assess patient conditions and provide safe care
- Basic computer skills and ability to learn facility computer programs
- Ability to work in a long-term care setting
