PRN Clinical Pharmacist

Role Description The Clinical Pharmacist is a part-time, on-site role based at Milwaukee Rehabilitation Hospital in Milwaukee, WI. This role is responsible for reviewing and verifying medication orders, ensuring safe and effective pharmaceutical care for patients with complex rehabilitation and medical needs. Day-to-day responsibilities include collaborating with physicians, nurses, and other members of the care team on medication therapy management, monitoring for drug interactions and adverse effects, and adjusting medication regimens based on clinical status and evidence-based guidelines. The Clinical Pharmacist will compound medications when necessary, maintain accurate documentation, participate in medication reconciliation, and provide education to patients and staff on appropriate medication use. The role also supports quality improvement initiatives and adheres to hospital policies, regulatory standards, and best practices in clinical pharmacy. Qualifications Candidates should possess strong skills in Pharmacy Practice and Pharmacy, with experience in hospital or clinical settings. Candidates should possess skills in Pharmaceutical Care and Medicine, including medication therapy management and clinical decision-making for complex conditions. Candidates should possess skills in Compounding, including preparation of specialized formulations in accordance with safety and regulatory standards. Current PharmD or equivalent degree from an accredited school of pharmacy. Active pharmacist license in the state of Wisconsin, or eligibility for licensure within a defined timeframe. Prior experience in inpatient or rehabilitation hospital pharmacy is preferred. Ability to work collaboratively in a multidisciplinary team, with strong communication and interpersonal skills.
