Part-Time Licensed Practical Nurse Hours Mon 1p-7p, Thu & Sun 7a-1p. Additional Per Diem shifts available. Patient Overview Elderly male with progressive dementia, severe fall risk. Uses wheelchair. Licensed Practical Nurse (LPN) Job Duties Perform care as defined by client’s individual Plan of Care and within scope of the state’s nurse practice act. Monitor patient’s condition and notify appropriate personnel of any changes. Provide those services requiring substantial and specialized nursing skill. Assure proper maintenance of clinical records in compliance with local, state, and federal laws. Counsel the client and family in meeting nursing and related needs. Position Requirements Nurse Qualifications Current and unrestricted LPN license in the state for which you are applying (Illinois). Home health experience preferred. Memory care experience required.
