Location: Elkhart, Indiana

Start Date: 08-05-2026

End Date: 11-04-2026

Contract Type: Travel Contract

Shift Type: Monday - Friday, 8:00 AM - 4:00 PM (Day shift, 40 hours/week)

Hours per Week: 40

Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Screen, evaluate, and treat patients to restore function and mobility
Develop and update individualized treatment and discharge plans of care
Document services accurately and timely to support continuity of care
Supervise Physical Therapist Assistants and Rehab Techs per state practice acts
Collaborate with interdisciplinary care teams and families to optimize patient outcomes
Maintain compliance with safety, infection control, and regulatory standards

Qualifications

Graduate of an APTA-accredited Physical Therapy program (DPT, MSPT, or BSPT)
Active Indiana state PT license (or ability to obtain prior to start)
Strong communication, documentation, and clinical skills
Flexibility to adapt to varying facilities, schedules, and patient needs

Education

Graduate of an APTA-accredited Physical Therapy program (DPT, MSPT, or BSPT)

Knowledge & Skills

Physical therapy evaluation and treatment planning
Patient evaluation and functional mobility assessment
Accurate and timely clinical documentation
Interdisciplinary communication and collaboration
Knowledge of state and federal regulations and facility guidelines
Adaptability to different rehab settings and patient populations
