We areseekinga compassionate and experienced OB Registered Nurse to join our team. RN will provide specialized care topregnant patientswithaverage-riskandhigh-risk pregnancies, working closely with clinicians and other healthcare professionals to deliver comprehensive prenatal, perinatal, and postpartum care. The ideal candidate willpossessa strong background in OB/GYN nursing, excellent communication skills, and a deep commitment to improving maternal and infant health outcomes in underserved communities. This role is on-site in Midtown - Sacramento, CA (95811). Essential Functions Provide direct patient care to average-risk and high-risk pregnant patients, including prenatal, perinatal, and postpartum care, in collaboration with perinatal service-providing clinicians and healthcare teams. Monitor and assess the health of pregnant patients, identifying complications or risk factors that may require specialized intervention or referral. Educate patients on prenatal care, pregnancy health, childbirth, common chronic conditions of pregnancy (including gestational diabetes and hypertension in pregnancy) and postpartum care, tailoring information to their specific needs and cultural background. Develop and implement individualized care plans for average-risk and high-risk pregnant patients, ensuring timely follow-up and coordination with other healthcare clinicians. Assist with diagnostic tests, including ultrasounds, non-stress tests, and other monitoring procedures to track fetal health and pregnancy progress. Administer medications, treatments, and prenatal supplements as prescribed, while ensuring patients understand proper usage and potential side effects. Provide emotional support and counseling to patients, addressing concerns about pregnancy, labor, delivery, pregnancy loss, and infant care. Document patient care, assessments, and interventions in the electronic health record (EHR) in compliance with clinical protocols and legal requirements. Collaborate with case managers, social workers, and community resources to address social determinants of health and ensure comprehensive care for patients with complex needs. Participate in health education programs and prenatal classes aimed at promoting healthy pregnancies, breastfeeding, and postpartum recovery. Facilitate care team meetings overseeing high-risk pregnancy cases. Maintain up-to-date knowledge of best practices in average-risk and high-risk obstetrics and gynecology through continuing education and professional development. Support comprehensive perinatal services program (CPSP/NEST) orientation appointments by collecting detailed patient health histories and ordering first-trimester labs (or labs based on gestational age at intake). Facilitate and support group classes on topics relevant to CPSP (NEST) patients, such as prenatal health, nutrition, and breastfeeding education. Assist in case management (including NEST patients), including tracking follow-ups, coordinating care with providers, and addressing patient needs throughout pregnancy. Current and valid Registered Nurse (RN) license in the state of California. Minimum of 2 years of experience in OB/GYN nursing, withexperience managinghigh-risk pregnanciesrequired. Strong clinical assessment and critical-thinking skills, with the ability toidentifyand respond to complications in high-risk pregnancies. Excellent communication and interpersonal skills, with a commitment to patient-centered care and health equity. Familiarity with electronic health record (EHR) systems and ability to accurately document patient care. Active CPR or BLS Certification Preferred Electronic Fetal Monitoring (EFM) Certification preferred. Experience or certification as a lactation consult or equivalent skill Bilingual in Spanish, Dari,Pashtoor another language spoken by the patient population is a plus. Experience in a Federally Qualified Health Center (FQHC) or community health setting is highly desirable. Ability to collaborate effectively across a broad spectrum of backgrounds and perspectives.
