• Rehab experience
• Familiarity with electronic medical records
• Experience with trachs, admissions, feeding tubes, and neuro patients
• Experience in a skilled nursing facility setting

Description:

As a continuing care LPN, providing top-notch patient care is paramount. You will be responsible for assessing, planning, implementing, and evaluating patient care under the direction of an RN or Physician. You will be a vital part of our team, making a real difference in the lives of our patients and their families. Collect and monitor assessment data and report physiological, psychological, sociocultural, and spiritual needs of patients and families. Provide direct patient care consistent with policies and procedures. Inform patients and families about treatments and medications being received and equipment being utilized. Document care provided in a manner consistent with guidelines. Communicate with patients and families in a caring and respectful manner. Collaborate with other members of the healthcare team to ensure continuity of care. Clean patient environment/work area/equipment and report any equipment malfunctions. Participate in selected quality monitoring activities under the direction of an RN or Physician. Demonstrate a high standard of moral and ethical behavior. Advocate for privacy, confidentiality, and security of patient and staff data. Actively participate in safety initiatives and complete all position and unit safety-related competencies. Perform other duties as assigned.
