Salary: $20.50 - $27.50 per hour

• Help individuals discover their strengths and set personal goals.
• Lead person-centered planning (PCSP).
• Navigate services like Day and Residential programs, Medicaid/MCOs, SSI/SSDI, SNAP, VR, mental health & medical support.
• Advocate for choice, rights, and independence.
• Coordinate services and build partnerships in the community.
• Balance meaningful relationships with important documentation and case management tasks.

Requirements:

• Bachelors degree and 6+ months experience with IDD/Intellectual and Developmental Disabilities OR equivalent experience in the field.
• Valid drivers license and good driving record.
• Ability to pass a background check and drug screening.
