Work Environment

This position primarily involves working in home care settings where you will visit patients at their residences in the Sunnyvale area. Here, you will enjoy a collaborative atmosphere that values teamwork and patient-centered care. Your work routine will consist of scheduled appointments from Monday to Friday, 9:00 AM to 5:00 PM, allowing you to maintain a work-life balance while making a significant impact in our clients' lives.

Key Responsibilities

• Deliver one-on-one patient care, managing 1-2 patients per hour to ensure personalized attention and effective treatment.
• Conduct thorough assessments to develop individualized treatment plans addressing each patient's specific needs and goals.
• Implement evidence-based physical therapy techniques, such as therapeutic exercises, manual therapy, and modalities, to optimize patient recovery.
• Maintain accurate and comprehensive documentation through our Electronic Medical Record (EMR) system, ensuring compliance and continuity of care.
• Collaborate with multidisciplinary teams, including physicians and occupational therapists, to coordinate holistic treatment approaches.
• Educate patients and their families on therapeutic exercises and home care strategies to promote ongoing recovery and wellness.

Patient Population / Client Focus

In this role, you will focus on a diverse patient population primarily dealing with orthopedic conditions. This includes but is not limited to post-surgical rehabilitation, musculoskeletal injuries, and chronic pain management. Your expertise will be essential in helping patients regain their mobility and improve their overall quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Active California Physical Therapy license or eligibility to secure one.
• Strong analytical skills for effectively modifying treatment plans based on patient progress.
• Excellent verbal and written communication abilities to facilitate patient engagement and education.

Preferred Experience

• A minimum of 2 years of experience in an outpatient orthopedic setting is required.
• Experience with EMR systems to ensure seamless documentation and patient management.
• Strong mentorship and communication skills to lead less experienced staff in our collaborative environment.
