Job Summary:
The organization is seeking a qualified Physical Therapist specializing in Home Health services in the state of California. The position involves providing patient-focused therapy in a home health setting, ensuring effective treatment plans and patient care for adult and geriatric populations.

Job Details:

• Location: LAKEPORT, California
• Schedule: Days, 8 hours per shift
• Minimum Hours Per Week: 40 hours
• Required Qualifications: Valid California Physical Therapist license, at least 1 year of experience as a home health physical therapist, valid driver's license, and auto insurance. BLS certification is preferred.
• Professional Skills: Proficiency in adaptive devices, gait training, patient education, assessments, and handling various orthopedics and neurological conditions. Experience with electronic charting and patient management in a community hospital or home health setting is preferred.
• Patient Care Details: Provides therapy services to adult and geriatric patient populations, with a patient ratio of 5 visits per day.
• Additional Information: The role involves carrying out evaluations, treatment plans, and patient discharge procedures in compliance with healthcare standards. Onboarding requires completion of required labs within 3-5 days of contract signing. Candidates must verify 7 years of work history and provide any relevant certifications with expiration dates.
